In all honesty, upgrades are very, very simple. I would suggest taking a backup of the database prior to the upgrade. You will need to login as the BES service account to perform the upgrade. Prior to an upgrade (or really any planned service restart or server reboot), I personally remove users who are 30+ days inactive and purge any "large" number (> 50) of pending messages (listed as pending data packets in BlackBerry Manager), as this tends to allow services to recover much more quickly post-restart (it performs a rescan on all mailboxes to gather any status changes for messages during the downtime).

As for the gotchas, they were pretty much worked out with the later 4.x service packs and maintenance releases, from my own personal experience. We upgraded to SP6 a little less than a year ago from SP4 with no issues to be reported.
